Immunization Information

The Indiana State Department of Health adopted rules under IC 4-22-2 requiring the school age child to receive additional immunizations against the following: Meningitis, Varicella, and Pertussis (whooping cough). What this means for your child is that before entry to school all 6^th-12^th grade students will be required to have 2 doses of varicella (chickenpox) vaccine, given on or after the first birthday, and separated by-appropriate intervals as defined by the CDC, or a history of chickenpox disease that has written verification by the parent or physician. All 6^th– 12^th grade students will be required to have 1 dose of tetanus-diphtheria-acellular pertussis vaccine (Tdap) given on or after the 10^th birthday. In addition, all 6^th-12^th grade students will be required to have 1 dose of meningococcal conjugate vaccine (MCV4).
